I am sure that many may disagree but I have personally witnessed a real big gap in practical solutions based on evidence and the stiff and ridiculous over-regulated rules that have been generated by those who do not even visit the field. My philosophy is that you take your example from the best, and modify it from there to meet your needs. IMHO, the best is the BLM, they are the standard. To that end , there is a BLM book that is all about field notes. I cannot tell you the name of it because my copy has the cover sheet torn off of it. But I would think if you googled for it, or called your local BLM office and asked for the Surveying Dept. they could tell you what it is.
